Incredibly important school vaccination news for parents who claim personal or religious exemptions from required school vaccinations for their children.
New law in effect.
Now you are required to use the Colorado Department of Public Health and Environment’s standard Certificates of Medical and Nonmedical Exemption.
Beginning the 2021-2022 school year, there will be 2 ways to file a nonmedical exemption.
- Option 1. Download the official Certificate of Nonmedical Exemption to be signed by an immunizing provider: Colorado licensed physician (MD, DO), Advanced practice nurse, or Physician Assistant, or pharmacist.
*Form can be found at https://cdphe.colorado.gov/vaccine-exemptions under downloadable Certificate of Nonmedical Exemption - Option 2 is to take an online Immunization Education Module.
This is available 24 hours/day and 7 days/week at https://www.dcphrapps.dphe.state.co.us/storyline360/story.html After completion of the course, an electronic form must be completed to obtain the Certificate of Nonmedical exemption.
You will have the option to complete a form for more than one minor in the family without having to retake the module.
